Folks with chronic health conditions and disabilities typically face many additional challenges in their daily lives. Chronic Conditions Collective Care (C4) is a space for everyone who lives with chronic conditions\, and their care-takers. We don’t offer unsolicited advice or share personal details regarding members. Our hope is to also see what needs we can address with mutual aid. \nQuestions?
